We do not bill insurance for couples therapy because most insurers require a specific mental health diagnosis for reimbursement. Couples work focuses on relationship patterns rather than diagnosing one partner, and assigning a diagnosis can affect confidentiality, legal records, and how you’re perceived by insurers. To protect your privacy and keep treatment focused on the relationship, we operate on a private-pay basis.

By law, our therapists can only see clients who reside in New York State with the following exceptions:

• Shahreen can see clients in New York , New Jersey and Florida.

• Daisy can see clients in New York, New Jersey, Connecticut, Florida, Pennsylvania and Texas.

• Amy can see clients in New York and Florida

• Alexandra can see clients in New York, Connecticut, and Florida

• Megan can see clients in Connecticut, Florida, New Jersey and Pennsylvania.

We only fill out FMLA and related forms for clients who we have been seeing consecutively for six weeks. Please note that there is a $125 admin fee for each form.

We do not write emotional animal support letters or fill out forms on this subject. Our legal team has advised us that writing these letters are not within our scope of practice.

Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.

This form may be used by the health care providers to inform individuals who are not enrolled in a plan or coverage or a Federal health care program (uninsured individuals), or individuals who are enrolled but not seeking to file a claim with their plan or coverage (self-pay individuals) of their right to a “Good Faith Estimate” to help them estimate the expected charges they may be billed for receiving certain health care items and services. Information regarding the availability of a “Good Faith Estimate” must be prominently displayed on the convening provider’s and convening facility’s website and in the office and on-site where scheduling or questions about the cost of health care occur.

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.

Yes, confidentiality is a top priority at Purposeful Living. We respect your right to privacy and maintain strict confidentiality of all client information and records. Our therapists are bound by ethical and legal standards to maintain confidentiality and will only share client information with others on a "need to know" basis or with your written consent. However, there are certain exceptions to confidentiality such as if a client poses a risk of harm to themselves or others, or if there is suspected abuse or neglect of a minor or vulnerable adult. In these cases, we are required by law to report the situation to the appropriate authorities. We encourage you to discuss any concerns or questions you may have regarding confidentiality with your therapist.

At Purposeful Living, we require that clients provide us with 24-hour advanced notification on all cancellations. Clients who fail to meet this criteria when canceling will be responsible for paying a $100 late cancellation charge, regardless of what your copay is. Insurance companies do not pay for missed sessions. However, we understand that emergencies can arise, and we will consider emergency cancellations on a case-by-case basis. It is important to note that we have this policy in place to ensure that our therapists' time is respected, and that we can continue to provide quality care to all of our clients.
